By visiting the Liver Foundation website, you are accepting and consenting to the practices described in this privacy policy. The purpose of this privacy policy is to set out the type of information that the Liver Foundation will collect and the purposes for which the Liver Foundation will use and/or disclose your personal information, which you provide to us.
We respect the privacy of the personal information you may provide to us when we deal with you. The way we manage your personal information is governed by the Privacy Act 1988 (Cth) (Privacy Act) and the Australian Privacy Principles (APPs) established under the Privacy Act. Please note that this Privacy Policy does not extend your rights or the Liver Foundation’s obligations beyond those defined in the Privacy Act. If there is any inconsistency between this Privacy Policy and the Privacy Act, this Privacy Policy shall be interpreted to give effect to and comply with the Privacy Act.
The information we collect from you allows us to personalise and improve the website experience. We may also use the information to communicate with you. We also use this information to prevent or detect fraud or abuses of the website and enable third parties to carry out technical, logistical or other functions on our behalf.

Sensitive Information is:
(1)personal information or an opinion about:
- racial or ethnic origins
- political opinions or political associations
- philosophical beliefs or religious beliefs or affiliations
- sexual preferences or practices
- criminal record

(3)genetic information about an individual that is not otherwise health information
This Privacy Policy includes examples but is not intended to be restricted in its application to such examples. Where the word ‘including’ is used, it shall mean ‘including without limitation’.
Collection and use of personal information
Your personal information will be collected by the Liver Foundation through the website and other sources where you have consented to us collecting your information. We will only collect personally identifiable information that you choose to provide. You may choose to provide us with personally identifiable information if, for example, you contact us with an enquiry, register on the website, fill out a survey or other form with your personal information included or request us to provide you with information. You consent for us to collect, use, disclose and store your personal information for the following purposes:

- to provide health advice, support and education to you
- to comply with our legal obligations
- to help us manage and enhance our services
- to assess, train, work with and manage our staff, volunteers and contractors

We may also collect other personal and sensitive information from you (for example information about your health, including medical history) to allow the Liver Foundation to properly assess and advise on your health care needs.

Personal information collected via the website
Every time you visit the website, the Liver Foundation may collect information about you, including:
- the date and time of your visit to the Liver Foundation website and program

- any address of a recurring site and any website you are about to visit
The above information provides the Liver Foundation with details about how the website is used including the frequency and duration of visits, and which web pages you have accessed on the website. The information we collect will not personally identify you.

Accessing your personal information
You have a right to request access to the personal information that we hold about you. The right to access is subject to certain exceptions allowed by law. Liver Foundation will, subject to applicable privacy laws, provide you with access to your information that is stored with us, and will deal with your request in a reasonable time frame, in most circumstances within 30 days.
Please be aware, in some circumstances the law permits Liver Foundation to refuse your request to provide you with access to your personal information. In addition to privacy laws, you may have rights to access your personal information contained in certain Liver Foundation Documents. You can apply for access through the Freedom of Information Act 1981.
If you believe that any information, we hold about you is incorrect, out of date, irrelevant or misleading we will take reasonable steps to correct this information once it is brought to our attention.

Consent
Providing your personal information to the Liver Foundation is optional. However, if you do not provide your personal information to the Liver Foundation, we may not be able to provide you with the full suite of services that are provided through the website. We also may not be able to contact you or give you access to information. By providing your personal information you are deemed to have consented to the collection, use, disclosure and storage of that information as outlined in this Privacy Policy.
Management of personal information
Your personal information may be stored in hard copy documents in locked filing cabinets, or in electronic format on a cloud-based database hosted in Australia. Access will be restricted to those within the Liver Foundation who require it in order to carry out their role within the Foundation.
The Liver Foundation treats breaches of your privacy very seriously and any breach will result in disciplinary action being taken. If a data breach occurs that involves unauthorised access to, disclosure of, or loss of personal information and this is likely to result in serious harm to any individual about whom the information relates, we will comply with the data breach notification scheme in the Privacy Act. The Liver Foundation ensures that our management of personal information is in accordance with this Privacy Policy and where applicable, the Privacy Act 1988 or the EU’s General Data Protection Regulation (the GDPR).
Health Records
If the information that we hold about you forms part of a health or medical record that we hold as your support service provider, that information will be held for a period of at least seven (7) years from the last time we provided you with a health service, in accordance with the NSW Health Records Act 2002 or other applicable laws. If the health record is that of someone under the age of 18, that information will be held at least until that person turns 25.
A note for those in the EU
If you are in the EU, then how we use your information may be governed by the GDPR and we will only use your personal data when the law allows us to and for the purpose for which we collected it which include the following:
- To manage your relationship with us
- To enable you to participate in activities or complete a survey
